New Drug Combo Helps Fatty Liver Disease — Even Before Weight Loss
Scientists also discover that pigment-producing skin cells may be "woken up" in people with vitiligo.
Scientists are reporting two promising medical breakthroughs this week. A new experimental drug combination showed strong results in treating fatty liver disease, and researchers in Japan found a possible new way to treat vitiligo, a condition that causes patches of skin to lose their color. Both discoveries could lead to better treatments for people who have had few good options in the past.
The fatty liver drug was developed by a company called D&D Pharmatech. It combines two experimental drugs — one called zabopegdutide, which belongs to a class of drugs known as GLP-1 drugs, and another called TLY012, which is designed to kill damaged cells in the body. Patients in the study were overweight or obese and had fatty liver disease confirmed by a biopsy, which is when doctors take a small sample of tissue to look at under a microscope.
After 48 weeks of treatment, patients who took the new drug combo had much greater reductions in liver fat than patients who took a placebo, which is a pill with no medicine in it. The results were published in a medical journal called the Lancet Gastroenterology and Hepatology. Doctors were excited to see real improvements in the tiny structures of liver tissue, which showed up in both biopsies and blood tests.
One of the most surprising findings was that improvements in the liver showed up after just 12 weeks — before patients had even lost much weight. Even patients who had lost less than 5% of their body weight still showed big reductions in liver fat and liver stiffness. The CEO of D&D Pharmatech, Seulki Lee, said this suggests the drug is helping the liver directly, not just by helping people lose weight.
The second story is about vitiligo, a condition where the immune system attacks pigment-producing cells in the skin, leaving white patches. Researchers from Osaka University in Japan found that these pigment-producing cells, called melanocytes, have not actually disappeared in those white patches. Instead, the cells have gone into a kind of 'sleep mode,' reverting to a more basic form and losing their ability to make pigment.
Normally, melanocytes attach to a thin layer of tissue called the basement membrane using a protein called laminin-211. In people with vitiligo, that protein is replaced by a different one called laminin-332, which causes the melanocytes to attach differently. This change triggers a chain reaction inside the cells that puts them into a dormant, or sleeping, state where they stop producing color.
When researchers tested drugs in mice and in human skin samples to target those pathways, they were able to 'wake up' the sleeping melanocytes and get them producing pigment again. Study leader Dr. Lingli Yang said this discovery 'uncovers a new mechanism underlying the development of vitiligo, which could change how we treat the disease.' Most current treatments only try to stop the immune system from attacking the skin, so finding a way to reactivate the cells themselves is a major new direction for scientists.
"Many of these benefits occurred before meaningful weight loss, suggesting that zabopegdutide provides direct therapeutic effects on the liver beyond weight reduction."
