Does Extra Weight Cause Depression? Scientists Find It's More Complicated Than We Thought

A new study shows that higher BMI is most strongly linked to changes in appetite, not to feelings of sadness or hopelessness.

Scientists have long noticed that people with higher body weight are more likely to feel depressed. But a new study published in the journal Molecular Psychiatry suggests the connection is far more complex than it looks. Researchers used a special method involving genetics to figure out whether extra body weight actually causes depression, or whether something else is going on. Their findings surprised even the scientists themselves.

The study focused on BMI, which stands for Body Mass Index. BMI is a number that compares a person's weight to their height. Doctors often use it as a rough measure of whether someone is at a healthy weight. For years, studies have shown that people with a higher BMI tend to report more symptoms of depression, but scientists weren't sure if the extra weight was actually causing the depression.

To dig deeper, the researchers used a technique called Mendelian randomization. This method uses genes — the tiny instructions inside our cells — as a kind of natural experiment. Because genes are passed down randomly from parents to children, scientists can use them to test cause-and-effect relationships in a way that is hard to fake. Think of it like a lottery: some people randomly receive genes that push their BMI higher, and scientists can study whether those people also end up more depressed.

The team looked at 324 different gene variants that are linked to BMI. They sorted these gene variants into six groups, or clusters, based on what other traits they were connected to. Some clusters were linked to body fat and metabolism. Others were connected to social and economic factors, like income and education. This sorting helped the scientists understand different pathways through which BMI might affect mental health.

Next, the researchers checked whether each cluster had an effect on nine specific depression symptoms. These symptoms came from a common depression questionnaire called the PHQ-9. The symptoms include things like feeling sad, losing interest in activities, having trouble sleeping, feeling tired, having trouble concentrating, and changes in appetite. Over 224,000 people's genetic data was used in the analysis, making this one of the largest studies of its kind.

The results were eye-opening. Across all six gene clusters, the one symptom most consistently linked to higher BMI was a change in appetite — for example, eating much more or much less than usual. This finding held up no matter which group of genes the scientists looked at, which made them confident it was a real effect. Changes in appetite are directly tied to how the body manages food and energy, so this connection makes biological sense.

However, the picture looked very different for other depression symptoms. Things like trouble concentrating, low energy, loss of interest in fun activities, and sleep problems showed weaker and less consistent links to BMI. When the scientists used the most careful statistical methods — ones designed to filter out misleading signals — many of these weaker effects disappeared. This suggested that those links might not be real causes, but rather the result of other hidden factors.

Most strikingly, the study found little to no evidence that higher BMI causes the emotional core of depression. Feeling deeply sad, hopeless, or having thoughts of self-harm did not appear to be directly caused by a higher BMI. This is a big deal, because many people assume that gaining weight makes people feel emotionally worse. The study suggests that connection may be much weaker than previously believed.

The scientists also found something important about the different gene clusters. The six clusters did not all point to the same answer. When results vary this much depending on which genes you look at, it often means the genes are affecting the outcome through other traits — not through BMI itself. Scientists call this horizontal pleiotropy, which means the genes are taking a detour through something else.

So what does all this mean for health and medicine? The study suggests that when earlier research found a link between BMI and depression, part of that link was driven by appetite changes — a physical, body-based symptom. But the emotional symptoms of depression, like sadness and hopelessness, don't seem to be directly caused by having a higher BMI. Doctors and researchers may need to look more carefully at which parts of depression are truly connected to body weight, and treat emotional symptoms separately.

BMI had the largest effect on appetite changes, which was robust across estimators, homogeneous across clusters, and survived correction for multiple testing.
